Chronic Disease


For most older adults, good health ensures independence, security, and productivity as they age. Yet many struggle every day with health and safety challenges such as chronic diseases and falls; both of which can severely impact quality of life. The Department of Health and Senior Services (DHSS) has developed a Chronic Disease Toolkit for Seniors. This toolkit provides education and resources for seniors on topics including preventing chronic diseases, arthritis/osteoporosis, asthma/COPD, diabetes, heart disease, stroke, Alzheimer’s, HIV/STD/Hepatitis, cancer, falls, and prescriptions.
